Medical Audit of Screening Mammography Performed in Women Over 40 Years Old at Seven Affiliated University Hospitals for One Year

Abstract
Purpose: To perform the appropriate medical audit in Korean experiences through comparison and analysis of the mammography performed at seven affiliated university hospitals.

Materials and Methods: We reviewed the results of the mammography at 7 screening centers in the 8 affiliated university hospitals between March 2004 and February 2005. All results were retrospect analyzed the parenchyma pattern and category on the basis of the ACR BI-RADS system. We obtained the positive rate or recall rate, cancer detection rate, PPV1, PPV2, stage 0 or 1 tumor rate, minimal cancer rate, and node positive rate. And we obtained recall rate, cancer detection rate, PPV1, PPV2 by age and parenchyma density.

Results: Recall rate was 16.02% (1.75-32.63%). Cancer detection rate was 1/1000 (0-2.84/1000). PPV1 was 0.64% (0-8.62%). PPV2 was 21.43% (0-55.56%). The rate of stage 0 or 1 tumor was 75%, minimal cancer was 50%, and node positive rate was 17%. The recall rate, PPV1, PPV2 of 40 years old women were 23.01%, 0.26%, 11.76% and those of over 50 years old women were 10.15%, 1.35%, 30.56%. The recall rate, PPV1, PPV2 of the fatty breast were 4.82%, 2.70%, 58.33% and those of dense breast were 29.91%, 0.37%, and 14.58%.

Conclusion: Recall rate of our results was higher than the desirable goal of ACR. PPV1, PPV2, cancer detection rate were lower than those of ACR, and there were some variation through each hospital. But, in over 50 years old women and fatty breast, the performance rates were relatively excellent. For quality assurance of audit data, education of the ACR BI-RADS system and establishment of the appropriate Korean audit data were needed.
